Memberships are not insurance, but a payment arrangement provided by All Smiles Dental Studio for services rendered*.
• Memberships are not transferrable to other dental offices or patients.
• Payment for any cosmetic or restorative treatment is due at time of service.
• Membership dues must be current to receive services.
• Necessary x-rays are determined by the doctor at time of exam.
• All plans require a $149 activation fee. Activation fees cover your first month's payment.
• Membership fees may be prorated for new members added to an existing account.
